Office Information:

Times available:

I offer afternoon and evening appointments from Monday-Thursday.

Payment options:

(a) For those without insurance: I offer a sliding scale fee based on household income, and a free initial consultation upon request, to those who do not have insurance or another third-party payment option. My sliding scale fee range is from $60-$120.

(b) For those with insurance: I am a member of the Blue Shield, Pacificare, UBH, and US Behavioral Health Plan panels, as well as a member of a few EAPs. If you participate in a different 3rd-party payment plan which reimburses you for visits with out-of-network providers, I will provide you with documentation of your fee payment, which you can submit to your insurance company in turn. If your 3rd-party payor does not reimburse you for visits with out-of-network providers, then the sliding scale alternative is available as described above.
